College experience preferred.SMCN is offering an increase after 90 days of employment (based on good performance).Southern Maryland Community Network is a non-profit organization that serves children, adolescents and adults with mental health needs in the Southern Maryland area. We are currently hiring for a full-time Rehab Case Specialist for our Prince Frederick office location. We provide community-based, mobile support services to individuals in the community. Rehab Case Specialists see the individuals on his/her caseload six times a month and will utilize a company vehicle and mobile device to document services. Services are provided in the home, out in the community or in the office environment. The Rehab Case Specialist helps individuals on his/her caseload with obtaining and maintaining entitlements such as social security disability income, food stamps, medical assistance, housing, etc. Individuals develop their own goals to help them reach recovery. The services we provide to mentally ill individuals in our community helps to reduce the number and frequency of incarcerations and hospitalizations. Our goal is rehabilitation, and our services help these individuals to maintain their independence within the community.Qualifications include: College Degree is preferred but a high school diploma is acceptable; experience is preferred but we are willing to train; must have a clean driving record with no more than two points; must pass a criminal background investigation and drug screening.Benefits include: medical and dental after 30 days of employment; paid short-term, long-term and life insurance; a 403 (b) retirement plan; wellness program options; 12 paid holidays annually; an employee assistance program; two types of paid leave accruals after 90 days; tuition reimbursement; professional development opportunities; employee events, etc.SMCN is an Equal Opportunity Employer.
