Job Description

Provides support and instruction on a 24/7 basis in an intensive care residential home in service to the medical, physical, psychological and social needs and personal goals for intellectually and developmentally disabled adults having more complex medical or behavioral needs. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, professional practices and including ethical standards related to client interactions.

Job Duties and Responsibilities

1. Provides instruction and guidance, assists and monitors residents in daily living skills and self-care such as hygiene, eating, mobility, money management, routine problem-solving , communication skills and community interactions and activities,
2. Makes supportive behavioral interventions.
3. Facilitates, networks and supports residents engaged in volunteer, social and leisure activities.
4. Assists residents with physician appointments.
5. Transports residents via agency van.
6. Dispenses and records resident medications.
7. Physically assists residents with mobility and basic daily living activities.
8. Liaises with family members and other concerned parties on behalf of the residents.
9. Greets visitors and supervises visitation to the facility.
10. Enforces observation of facility/house rules.
11. Monitors individual service plans in collaboration with resident; his/her family, legal guardian and/or legal representative; other service providers and the agency treatment team under supervision of the ICF Supervisor.
12. Observes, records and reports residents' daily progress, behaviors and incidents.
13. Prepares individual service plan documentation,
14. Provides overnight awake support, assistance, monitoring and supervision to residents.
15. Participates in housekeeping and meal preparation activities.
16. Secures the facility per policy.

Position Status

Required Qualifications

Considerable knowledge of the practices and methods related to the instruction of intellectually disabled adults including behavior management, independent living skills training and supportive counseling; considerable knowledge of human rights of the individual; considerable knowledge of common housekeeping practices; some knowledge of the operation of electronic medical records and associated documentation; ability to physically assist residents; ability to prepare accurate resident records; ability to establish rapport with SMI individuals and their families; ability to establish and maintain an effective working relationship with other staff; ability to communicate effectively orally and in writing; ability to function calmly in stressful situations; ability to observe, assess and record individual behaviors and functioning; ability to operate a vehicle; ability to effectively respond to crisis situations both behavioral and medical.
High School Diploma/ GED . Valid Virginia Driver's License.

Preferred Qualifications

Supplemental course work in the Behavioral Sciences and experience supporting individuals with intellectual disabilities preferred.

Physical Demands

Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Work is performed in a well-maintained group facility. The incumbent may work directly with participants experiencing psychological or emotional disorders and who may become aggressive or abusive. Assisting participants who may be of limited mobility skills and the maintenance of the facility require lifting and overall good physical condition. The incumbent is occasionally interrupted by work-related calls during off hours. Assistance in residential programs may require the incumbent to work in homes with varying degrees of accessibility. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to, a personal computer, calculator, copier, and fax machine. Significant walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling. Ability to lift or support individuals weighing in excess of 180 lbs. Ability to use THEROPS style techniques as needed. Ability to operate accessible van with chair lift. Ability to transfer individual from wheelchair to bed/toilet/chair with assistive devices. Ability to intervene with individuals who may exhibit aggressive behaviors.

Shift Type/Hours Worked

The successful candidate must be able to work a rotating shift to include evenings, weekends, and occasional overnights and holidays.
