Job Description Job Description Position Overview: Title: RN Case Manager Company: Chase Brexton Health Services Location: 1111 North Charles St, Baltimore, MD 21201 Salary: $40.00 - $43.00 per hour Schedule: Monday-Friday / 8:00 AM - 5:00 PM Includes one late evening shift per week (10:00 AM - 7:00 PM or 11:00 AM - 8:00 PM) Position Summary: The RN Case Manager provides professional, patient-centered nursing care in an ambulatory care setting at Chase Brexton Health Services, working under the guidance of the Lead Nurse. This role delivers both direct patient care and telephonic support to patients and their families, ensuring care is aligned with Ambulatory Care Nursing standards and organizational policies. The RN Case Manager plays a key role in coordinating multidisciplinary care, supporting positive patient outcomes, and promoting continuity of care across the healthcare team. Key Responsibilities: Collect, assess, and analyze patient health data to inform individualized care planning. Assist in the development, implementation, and ongoing evaluation of personalized care plans in collaboration with multidisciplinary care teams. Monitor patient progress and outcomes, adjusting interventions as needed to meet clinical goals. Provide patient education, coaching, and ongoing support to promote self-management and adherence to care plans. Communicate care interventions, updates, and outcomes clearly to patients, families, and care team members. Document all nursing activities accurately in electronic medical records in accordance with professional, organizational, and regulatory standards. Delegate appropriate tasks to licensed and unlicensed team members while ensuring patient safety and quality of care. Ensure compliance with applicable regulatory and accreditation standards, including Joint Commission and Patient-Centered Medical Home (PCMH) requirements. Participate in quality improvement initiatives and support efforts to enhance clinical workflows and patient care delivery. Required Qualifications: Current Maryland Registered Nurse (RN) license Graduation from an accredited nursing program Active Basic Life Support (BLS) certification Minimum of one year of ambulatory care experience Preferred Qualifications: Bachelor of Science in Nursing (BSN) Experience working with electronic medical record (EMR) systems Strong clinical assessment, critical thinking, and care coordination skills Excellent communication and patient education abilities If you have any questions about the information provided above, please feel free to apply to this posting and I will follow up with you shortly after to discuss the role and provide you with a more detailed job description.
