Nurses. This full-time Hospice RN role serves patients across
Eastern Pennsylvania, offering extensive clinical support,
manageable caseloads, and leadership committed to providing
outstanding care and supporting the nurses who provide it. A Day in
the Life of Our Registered Nurse Experience the joy of providing
compassionate hospice care, working alongside a devoted
interdisciplinary team. Design and oversee tailored hospice care
plans Offer emotional and clinical support to patients and their
families Document patient care in EMR systems (familiarity with
TruChart is a plus!) Coordinate care services throughout Eastern PA

team Requirements Our Ideal Candidates for Connection Active RN
license in Pennsylvania 2 years of RN experience (hospice/home
health preferred) Ability to work independently with clinical
support Experience with EMR systems Valid driver’s license,
transportation, and auto insurance Compassionate, organized,

$75,000–$80,000 per year, with a $10,000 First-Year Success Bonus
