Job Description:

Case managers offer hybrid linkage and support services to adults who have been diagnosed with a mental illness. Case Managers work with individuals an average of twice per month to help them meet their goals.Services/Goals may center around: housing, financial assistance, energy assistance, medical or mental health care, transportation, benefits and entitlements, substance abuse services etc...Case Managers will utilize a company vehicle and mobile device to provide services to individuals virtually, in the community or in the person's home. Digital documentation of assessments, treatment plans and visits is completed daily.A Bachelor's Degree in the Human Services field and one year of related experience is preferred but we are willing to train. Candidate must have a clean driving record with no more than 2 points along with the ability to pass a criminal background investigation and drug screening.SMCN offers an excellent benefits package.
